A caregiver was observed grabbing and dragging a child across the playground. The child was not standing on two feet.

This standard was found deficient as part of a DFPS investigation when a caregiver did not use good judgment when pushing a large stroller through a small doorway, causing a child in care to sustain an injury.

During a DFPS investigation it was found a caregiver became frustrated and proceeded to forcefully pick a child up and sat the child down causing the child's head to hit the wall.

During an investigation, it was found that an allegation of prohibited punishment was not reported to licensing after the operation staff was notified. I spoke with the operation staff about when and how to make a report.

Observed a 5 year old child in the same room as an infant who is 8 months old. There was more than an 18 month gap between the youngest infant and the 5 year old. This was corrected when the 5 year old was taken to their classroom.

The operation staff was notified of concerns of prohibited punishments and did not make a report which is required within 2 days of the allegation being made. This was corrected when I spoke with staff about reporting and how to make the report.

Observed one child without a health statement on file. I spoke with the director regarding when the health statement is needed.
Observed multiple rooms without dates on their activity plans or missing the activity plan. I spoke to each caregiver and the director about the activity plans and having dates on the plans.
Observed a bottle and a breast milk bag with no child's name on them. This was corrected when the bottle and breast milk bag were labeled.
Observed 8 staff files, one caregiver only had 9 hours of training for the 2023-2024 review period.
Observed a classroom with 7 infants ages 11 months to 18 months old with one caregiver. The specified age for this classroom was 12-17 months and the ratio is 1 caregiver for 5 children. The classroom was over ratio by 2 children. This was corrected when another caregiver was added to the classroom.
During an investigation it was found that a child was left unsupervised inside of the classroom for approximately 1 minute.
Observed an infant sleeping in a crib with a drop down gate not in the locked position. This was corrected when the caregiver lifted and locked in the drop down gate.
Observed two bottles in cribs. One infant was awake in the crib with a bottle and the other infant was asleep in the crib and there was a bottle in the crib with the infant. This was corrected when the bottles were revmoved from the cribs.
Observed a caregiver with less then 2 years of prior childcare experience without preservice training.
During an investigation it was found that caregivers did not properly supervise the children in care when multiple incidents occurred in the same classroom. Children have thrown items, pushed tables over, thrown other children's items, hit, kick and push other children on several occasions.
The operation did not have a current sanitation inspection. The last inspection was conducted August 2023.
Observed Tylenol that was expired in the medication cabinet
Observed a can of Paint under a sink without being locked. The paint was assessable to the children in care. This was corrected when the cabinet was locked.
Observed the black boarder of the playground with a crack in the plastic which can cause piching injuries. This was corrected when the operation taped over the crack on the plastic. Observed the fabric weed barrier coming up from the mulch causing a tripping hazard. I spoke with the person in charge regarding this concern.
The operations last fire inspection was in 2021.
Observed avtivity plans without dates of when the activity plan is for.
During an investigation it was found that on a day that two classrooms did not have water, the caregivers were using baby wipes to wash the children's hands.
Observed 1 child in a highchair without being strapped into the seat. The safety strap was missing from the seat. The caregiver put the child in a highchair that had safety straps.
It was found that caregivers are using thier phones while supervising the children in care. I spoke to the director about cell phone usage.
It was found that a child was left outside without supervision.
It was found that the operation failed to report a child being left unattended ouside and they also failed to report suspected abuse. I spoke to the caregivers and director about being mandated reporters.
Observed 2 children in highchairs without being strapped in the seat. The safety straps were missing from the seats.
During an investigation it was found that a caregiver used a cleaning agent on the table where children were eating.
The operation had purchased 2 vehicles that are used for transportation and the child safety alarms had not been installed.
Observed multiple rooms without an activity plan to review. This was discussed with the caregivers and director.
The builidng grounds checklist had not been completed since September 2022. This was corrected when the check was completed and the form was filled out.
3 classrooms caregivers did not know how many children they had in care. The first caregiver stated they had 6 children, but had 8 children in the class. The second classroom caregiver stated that they had 8 children in care but had 7 kids. The third classroom caregiver stated that they had 9 but had 10 kids. This was corrected when I spoke to the caregivers about knowing how many children they have and speaking to the person in charge about ways to help caregivers remember their count of the children.
Observed trim coming off of cabinet doors, rust on the bathroom door frame that was pealing, observed some of the cabinets/sink area that had paint and built up dirt on it.
Observed chemicals in a room under a sink that was not locked. The classroom is not being used at this time. This was corrected when the chemicals were removed.
Observed bottles without lables of the children names and observed some bottles with only initials. This was corrected when the bottles were labeled.
