A caregiver on the basketball court and in the toddler room were found using their personal phones while in direct supervision of the children.

Large climber on the playground had a 1 inch gap on each side of the top of the stairs where two pieces of the equipment meet that could cause a possible entrapment. Several anchor posts are also missing safety caps on the top of the climber prohibiting children from putting their hands and fingers inside.
During nap time, 3 classrooms were left unattended (Older School-age, Younger School-age and 4/5 yr old rooms) while the three caregivers were in the office.
A brown climbing unit and the large climbing unit under the awning on the large playground did not have the required soft surfacing depth around the equipment. The brown unit had 2 inches of depth on one side of the unit and 0 inches on the other side. The large climber has a 12 inches of pea gravel directly under the slide and less than an inch of pea gravel around the rest of the unit.
A red climbing car on the large playground has two tires that have come loose and have exposed screws. A Little Tykes slide on the smaller playground has a brace underneath that is not secured, causing the unit to rock. A brown climber on the large playground has a cracked area on the platform. There are numerous pieces of a new climbing unit being stored on the playground that children could potentially climb on and be injured.
Two rocking toys and one stationary metal car on the playground had exposed metal anchors which could result in injury if a child should fall upon the metal.
A caregiver left their classroom to greet the inspector at the front door, relieved the person in charge in their classroom and then returned to their classroom. The caregiver left the children in her care unattended during this time.
3 out of 10 child files evaluated did not contain an address for the child's emergency contact person listed on their enrollment form. NOTE: The director was able to contact the parents and obtained the missing addresses.

One caregiver in the Prek room (4 year olds) and one caregiver in the School-age room (ages 6-10 yrs) were in direct care of the children without an eligibility letter on file. The caregivers were inactivated in 1/19/22 and 9/16/21. NOTE: The caregivers were sent home from the building and a background check was submitted by the director.
The 3 year old classroom was observed sending a child to a restroom located outside of the classroom. There was one caregiver present with 10 children and the caregiver walked out of the room to assist the child in the restroom leaving the classroom unattended.
1 of the 2 vehicles utilized to transport children had a fire extinguisher that was registering empty.
2 out of 8 staff training records reviewed were missing documented training on emergency preparedness, preventing the spread of communicable disease, preventing and responding to emergencies due to food allergies, understanding building and physical premisis safety and handling, storing and disposing of hazardous waste.
There was a Little Tykes play house on the large playground that had several panels that were no longer connected to each other. There was also another play house on the smaller playground that had two panels that were not connected at the base of the house.
The operation's parent handbook did not contain information regarding the promotion of active play.
2 out of 10 child immunization records reviewed did not contain updated immunization records. One child, 3 yrs old, was missing their MMR and Varicella vaccines. One child, 4 yrs of age, was missing their Varicella vaccine.
