There was also a gap of more than 3 inches where slats of the fence do not meet the ground and create an area where children can attempt to climb under resulting in exiting to the street. Lower playground has a playhouse where the front of the house has come unattached from the house causing a possible entrapment or pinch hazard.
Room E and the 3-4 year room both had cords hanging from a mounted television and Christmas lights that were within reach of the children. Upper playground had exposed wires on the ground leading from the building to the electrical and cable boxes accessible to the children.
The operation's vehicle fire extinguisher was last inspected in 12/2020.
The upper playground had a section of a wall that is used as part of the fenced area that was missing and does not meet the 4 foot height requirement. This creates the ability for children to climb over and exit the playground.
