A caregiver was observed changing a child and walking away from the changing table, removing their hand from the child on several occasions.
It was observed that several children were changed and the changing surface was not sanitizing.
There is evidence to support that there was one staff member with 20 children that had a median age of 3 years old. The ratio for 3 year old's is 15 to one.
There is evidence to support staff not washing their hands or the children's hands according to the procedures.

Based on the evidence obtained during the investigation a caregiver did not use good judgment or self-control when disciplining children in their care when pulling the children by the ears as a form of discipline.
Screen time was being utliized with infants in the classroom.
The toddler classroom was observed eating snack and none of the children washed hands prior to snack time. An infant was observed having a diaper changed and their hands were not washed. The infant has the motor function to hold their head up and reach forward to a hand washing sink.
A toddler was observed upset and reached out for the caregivers attention multiple times. The caregiver did not speak to the child, did not hold the child and did not attempt to comfort the child at all. The caregiver ignored the child.
A caregiver did not wash hands after wiping a child's nose. A caregiver was observed changing multiple diapers and did not wash her hands in between any of the diaper changes.
A toddler was observed walking around with a training cup for over ten minutes. The caregiver did not attempt to have the child sit down while drinking from the cup.
Based on the evience obtained during the investigation, there were 16 children combined with infants, toddlers and pre-school age children and there was more than an 18 month age group between the youngest infant of 6 months of age and the oldest child in the group, a four year old.
