During a walk through an infant was sleeping in a restrictive device. There was another infant present in a crib that was not bare. This was fixed when the infant was removed from the restricted device and crib was made bare.

While watching a video a child was seen standing in timeout for approximately 8 minutes. After caregiver interviews it was found a caregiver displays prohibited punishment by yelling at children in care.

During caregiver interviews it was found that a child was left in a room unsupervised.

It was found the operation did not report a incident in a timely manner.

The background checks for one subject has been pending eligibility since 4/7/2022, which is 49 days past the initial submission. The last validation of the Background Check list was on 5/10/2022. *This was corrected at inspection when the subject was removed from the list.

There were unmounted fans in 4 classrooms. This was corrected at inspection when the fans were removed from the classrooms.

There were no background checks for a person listed as the Governing Body-Designee. *This was corrected at inspection when the initial background check was submitted.

During the investigation it was found that the operation did not use good judgement when they did not communicate to the parents that the afterschool program was closed and that transportation would not be provided that day.
