Feeding instructions for a 3 months in care were not available during the inspection.

The employee list which was not posted needs to be updated. The list did not include the name of an employee who has been at the center since December 2021.

Two of four files reviewed was missing information whether or not consent was given to transport in case of an emergency. One of four files was missing information about whether or not a child has allergies. The address for the emergency contacts was not included in the admission information.

One file reviewed was missing an affidavit of employment.

The operation failed to notify licensing of the added outdoor playground.

Observed two caregivers supervising a group of children between the ages of 12 months to 3 years old while they were eating breakfast and observed 1 child (10 years old) alone in a separate classroom unsupervised while doing virtual learning at the time of inspection.

One non-mounted/safeguarded electric fans was in use inside the toddler classroom and accessible to the children at the time of inspection.

Children were served pancakes, milk, water, and bananas and as per menu states bean and chorizo, flour tortilla, tomatoes, milk, and water. NOTE: CORRECTED AT INSPECTION. Menu was updated.

Transportation vehicle's first aid kit was incomplete. Missing thermometer, scissors, tweezers, and first aid guide.

Observed an infant bottle without being labeled with the infant's first name and initial of last name. NOTE: Corrected at inspection as name and initial of last name was added at the time of inspection.

The operation has not conducted a sheltering drill for severe weather at the time of inspection.

Fire drills for January 2021 and February 2021 were conducted, however the exit time was not documented at the time of inspection.
