Annual health inspection is not current. Last conducted on 12/27/21.

At the time of inspection one after school classroom is being used as a storage room and 2 broken little tyke were accessible to the children along with boards with nails.

Reviewed children records: four child are missing the health statement completed by their Dr.

Four out of eight files reviewed were missing: Emergency Contact information, and health care information.

Observed 1 caregiver supervising 6 infants ages 5 month, 6 months, 10 months, 10 months, 12 months, and 12 months old. The specified ages for the group was 0-11. The maximum 1 caregiver can have is 4; therefore, caregiver was over by 2 children.

Observed a 5 month infant asleep in a swing at the time of insepction .

Four of the eight children present were infants. There was only one caregiver. A second caregiver arrived at the end of the inspection.

A child had an injury on her face at the time of pickup. The caregiver claimed not to notice the injury while she was in care.

The sign in and sign out tracking system has incomplete information. Information required in the tracking system includes: the name of the child, the date and time of arrival and time of departure, and the parents or employees initials.

Three boards in the playground fence need to be repaired.
