In the infant room, caregivers were using restrictive devices such as bouncers to get children to go to sleep. One child was observed already alseep in the bouncer. This was corrected at inspection when the caregiver removed the child from the bouncer.

One child in care with a diagnosed allergy did not have an emergency allergy plan signed by the physician and the parent. The child in care has a epinephrine injector on-site for anaphylaxis.

The date on the fire inspection was 1-11-22.
There was a mixed age group of 4 year olds - school age children present and there was no current activity plan.
Ants were observed on the changing pad on a diaper changing table. The table was also dirty.
A new hire was present and providing unsupervised care, but there was no personnel records available for review of this employee.
The refrigerators were observed to have food debris and have not been properly cleaned. There were food items and bags left open and not properly covered. The counters were dirty, and showed signs of not being properly cleaned.
Water and milk were not served with the meal, until asked to do so by this inspector.
Three school age children present who were not in the sign-in/out system and there was no documentation showing they have been signed in. This was corrected when the director entered these children into the system.
The outdoor play area needs large weed clumps cut down. There are large briar vines ine the play yard that need removed. There were large trash bags full of weeds, leaves and other trash item and these bags were open and accessessible to children in care.
Spray cans of lysol and another chemical were observed sitting on a shelf and accessible to children in care. This was corrected when these items were put out of reach of children in care.
The employee did not reflect new employees and employees that have left employment.
A caregiver was observed not washing her hands or the child's hands after a diaper change. This was corrected when she washed her hands at request.
It was determined through the DFPS investigation, a child in care ingested craft beads when the caregiver was helping other children with the craft.
During the course of the DFPS investigation, it was determined an infant had access to craft beads.
The infant caresheets have not been updated since April 2021.
It was determined during this investigation that a caregiver did not properly supervise and redirect the children in care. The caregiver is no longer employed at the operation..
