Upon reviewing the staff application it was discoverd there were no documenttaion showing the references were contacted.

The service plan of one child referred to the child as the wrong last name on at least 3 occasions. Also, the plan was dated to be completed on 3.2.24, however, all signatures were dated 2.2.24, the child's admission date.

Two of the four children's files reviewed did not have contraindications of restraint documented in the admission assessment.
