While doing a walk through in a gym area used by children there were no safety plugs in the outlets. This was corrected at the inspection when safety plugs were installed.

During a walk through an infant under the age of 12 months was in a crib sleep with a bib on.

While doing a walk through children were seen eating food off of an ottomon. This was corrected at the inspection when children were placed in a high chair.

During a walkthrough there were cans of lysol and bleach water within reach of children. This was corrected at the inspection when the chemicals were put out of reach of children. During the walk through rooms used by children had dust buildup around the lights and on the ceiling.

Observed a wooden board missing on a platform that can cause a child's leg to be injured if they fall through. Observed two nails sticking out of the wooden structure on the playground. Observed a large nail that tacks down the perimiter of the playground that had came up, causeing a tripping hazard.

A 5 month old infant was observed asleep in their crib with 2 blankets inside the crib. The blankets were removed from the crib during inspection.
A caregiver was observed on their phone while supervising a group of children during naptime.
Three caregivers did not have documentation of annual training in the following areas: (1) Emergency preparedness; (2) Preventing and controlling the spread of communicable diseases, including immunizations; (3) Preventing and responding to emergencies due to food or an allergic reaction; (4) Understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic; and (5) Handling, storing, and disposing of hazardous materials including compliance with 746.3425 of this chapter (relating to Must caregivers wear gloves when handling blood or bodily fluids containing blood?).
The caregivers did not have in person training with use of manikin. The caregivers had completed online training for First Aid and CPR.
A caregiver was observed in a classroom supervising a group of children and did not have a background check on file. The Director sent the caregiver home and sent another caregiver into the classroom.
Based on the preponderance of evidence, it was determined parents previously informed to drop off their child in the foyer and a caregiver would take the child to their classroom. The operation has since allowed parents to access the classrooms and drop their children off there.
Two classrooms being used by children did not have smoke detectors.
