A child was injured when a piece of furniture that holds dress up clothing fell on a child when they tugged on the clothing hooks. The daycare removed the piece of furniture from the classroom and removed other ones like it from other classes in the building.
A caregiver did not provide proper supervision when they did not intervene in a situation to ensure a child's safety. The child was injured and had to seek medical attention.
