During an investigation it was found that a child was not sent home when the child had symptoms of a possible severe illness. There was a concern for the child's breathing and the child was not sent home.

A parent was not notified of a minor injury their child recieved while in care. I spoke with caregivers regarding checking the children if they start crying to ensure that no injuries occurred and to always notify the parents of minot injuries the day of the injury when the parent picks the child up.

Observed a child walk out of a classroom to use a bathroom in the hallway. One teacher left the classroom and closed the door behind them and walked to the front of the building leaving the child in the bathroom with no visial, or auditory supervision. Licensing spoke to the person in charge and they returned the child to the correct classroom.

During a follow up inspection an infant was seen sleeping in a crib with a small teddy blanket. This was fixed when the teddy blanket was removed.

While doing a walk through there was an infant in the crib sleeping with three boppy pillows hanging on the crib. There were also two infants sleep in restrictive devices. This was fixed when the infants were laid down in the cribs and the other infant was taken out of the crib with the boppy pillows.

Out of 10 staff files reviewed one was missing required annual training in abuse and neglect, SIDS/shaken baby, emergency preparedness, communicable disease, food allergy and hazardous materials.
Out of 10 staff files reviewed two were missing proof of education, one was missing employment affidavit, and copy of their photo ID.
Out of 10 child files reviewed 5 were missing completed emergency contact information and one was missing completed release information.
The stationoary fire truck and track bikes do not have a six feet use zone for either equipment.
There was one medication at the operation not in the original container. The medication was not labeled with the child's first and last name or date brought to the operation. *This was corrected at inspection when the parent was called and the medication was sent home. The operation did not administer the medication.
There are tools in the gym area accesible to children.
The fence around the toddler play ground is not complete.
