Observed lunch time and no drinking water was provided for the children.

Observed a tablet in the infant room with an infant sitting in front of it playing cartoons. This was corrected when the caregiver was spoken too and the tablet was turned off.

There were no activity plans for review for any of the classrooms.

Observed a broken plastic basketball goal. The plastic bottom was broken exposing sharp edges.

Observed the fire drill has not been conducted since March 2023. This was corrected when a fire drill was completed and the children were able to exit it under 3 minutes.

Observed a child in care without enrollment aggreement. I spoke to the person in charge about having the enrollment form completed.

Observed a child being left in the operation when a fire drill was being conducted. All other children and caregivers were outside while one child remained inside. The caregivers could not see, hear or be able to entervene if the child needed.

Observed an infant in a crib with a bottle. This was corrected when the caregiver was spoken too and the bottle was removed.

Observed the last fire inspectin to be conducted on 5/2022.

Observed a portable sink that was not turned on or have water. The children were having their hands washed with wipes. I spoke to the caregiver about washing hands with soap and running water. Wipes cannot be used to wash hands.

Observed an infant under 12 months old in a crib with a blanket. This was corrected when the caregiver was spoken too and the blanket was removed.

Observed one caregiver caring for children ages 14months to 22 months old. The specified age group for the class is 12-17 months, which put the caregiver out of ratio by 2 children. This was corrected when 2 children were moved into another classroom.

The last fire drill documented was completed in March 2022. A fire drill was completed and the staff and children were able to exit the operation in 1 minute and 50 seconds.

Two infants were sitting in a highchair without safety straps being secured around the infant. This was corrected when the children were removed from the high chairs.

Observed a person at the operation, with access to the children without a background check on file with the operation. I spoke to the director about the background check being entered. This was corrected when the person left the operation. An administrative penalty will be recommended as a result of this citation, per HRC 42.078. It was discussed to routinely check background check statuses on provider portal to ensure everything is current.

Observed 4 children records, none of the files had immunizations.

Observed two slides without unitary surfacing under or around the slides.

Observed children under the age of two in a classroom with cartoons on the television. This was corrected when the television was turned off.

In one classroom a 9 month old child was in the same classroom as a child who is 28 months old. I explained to the caregivers that there cannot be more than an 18 month gap between the oldest child and the youngest infant.

The fire drill had not been marked completed since July 2021. This was corrected when a fire drill was completed and the staff and children were able to exit in under 3 minutes.
