Two staff files did not have photo ID when reviewed.

A caregiver did not know the ages of the children in their care in the classroom.

There was one caregiver with seventeen children in the classroom with a specified age group of three years old.

A caregiver did not know how many children were in their care when asked by CCR.

The smoke detectors were not documented as being checked the previous month.

A caregiver did not have training in emergency preparedness, preventing communicable diseases, administering medications, preventing and responding to emergencies, understanding building and premises safety, and hazardous materials in the past training year.

The operation at time of inspection did not have the playground checklist filled out for the previous month.

The daycare did not maintain supervision of an infant was able to get into a toilet. *The daycare thoroughly cleaned the infant up*

An incident report was not completed for a situation that placed a child at risk.

Evidence gathered during the investigation indicated there was reason to believe that the director and the cook have pulled children by their arm while disciplining their behavior.

The operation demonstrated poor judgement in providing invoices for extermination treatments that did not occur.

The most recent fire inspection report was not available for inspection. The last inspection was conducted on 3/1/23.

Two out of ten child records evaluated did not have a health statement on file from the child's doctor ater being in attendance for 12 mos.

The 2 yr old classroom had 12 children and 1 caregiver. An infant caregiver was observed supervising 6 children, 9-18 mos of age.

A 4 yr old child was observed using the restroom that was 20 ft away from the classroom and adjacent to the kitchen while the caregiver stood in the doorway of the classroom.

The 2 yr old classroom was observed coming in from outside and children did not wash their hands.

One out of the 10 children's records evaluated was missing numbers for the authorized pick up. NOTE: This was corrected at inspection when the numbers for the authorized pick up was provided and updated.

During a teacher rotation, a child was left in a soiled diaper for hours at the operation.

Four infant feeding forms were found not updated.

An expired medication was found dated 11/2022.

Four year old classroom was found out of ratio with 22 children and one caregiver.

Infant tylenol, teething gel, and anti gas medication was found without the name of the child it pertained to.

The loose-fill around the play structure equipment on the Pre-K playground did not have the required depth of loose-fill available in the use zone. The loose fill was less than 1 inch in some areas.

Re-cite: Three year old classroom was found with 15 children in care, based on the space computation the max capacity is 13 children.

Two children's files reviewed did not have a current immunization record for review.

Infant classroom was found with ten infant cribs side by side without the necessary space between each.

Three year old classroom was found with 21 children in care, based on the space computation the max capacity is 13 children.

Three-year-old classroom is currently being used for storage and not maintained as licensed space.

A caregiver in the 3 year old classroom was overhead telling a child, "You do not move," in a loud voice. This was corrected when the person in charge went into the room to assist the caregiver and discussed positive methods of redirection.

The last fire inspection was conducted in May 2021.

At the time of inspection, the 0 to 11 month old classroom was not maintaining the child / caregiver ratio when there were six children and one caregiver. The 11 to 24 month old classroom was not maintaining child / caregiver ratio when there was five children under 17 months and a total of nine children with one caregiver.

At the time of inspection the caregiver in the three and four year old caregiver did not know how many children were in the group.

Based on the information gathered, the center staff failed to call in a report to the local licensing office when a child was injured and required medical treatment.

Investigation information obtained from DFPS Child Care Licensing investigator determined the caregiver gave a child a cough drop without parent medical authorization.

Investigation information obtained from DFPS Child Care Licensing investigator determined a caregiver did inappropriately grab a child by the arm to place the child in time out.

During the course of the investigation, it was determined that a child with a history of running, was not appropriately supervised which allowed the child to leave the playground and walk to the parking lot.

A new twos classroom was observed to be missing a daily schedule. Note: This was corrected at inspection when the director posted a daily schedule.

A heater was observed to be atop a low shelf in the 18-24m classroom. The heater was plugged in, off and was not mounted. Note: This was corrected at inspection when the director removed it from the classroom.

The nursery room was observed to have 5 infants with 1 caregiver. The infants ranged in age from 2 months-10 months. Note: This was corrected at inspection when the director moved a child into another classroom.
