At the time of inspection the caregiver in the three and four year old caregiver did not know how many children were in the group.
Based on the information gathered, the center staff failed to call in a report to the local licensing office when a child was injured and required medical treatment.

A heater was observed to be atop a low shelf in the 18-24m classroom. The heater was plugged in, off and was not mounted. Note: This was corrected at inspection when the director removed it from the classroom.
The nursery room was observed to have 5 infants with 1 caregiver. The infants ranged in age from 2 months-10 months. Note: This was corrected at inspection when the director moved a child into another classroom.
