Two caregivers were observed on the playground grabbing a child each by an arm and pulling the child across the outdoor activity space. The child was upset and then the child was held in a caregiver's lap and held in place for over10 minutes while they continued to be upset.

All three outdoor playgrounds had various hazard identified at inspection. There were nails protruding from the fence line. The nails were hammered in at inspection. Some pickets are broken and present a pinch hazard. A piece of metal is protruding from the ground. A brick was placed over it at inspection. A dramatic play equipment that has tires has wired mesh exposed with sharp edge. The equipment also has a loose board. The barrier to the climbing equipment has cracks. And a bench that child use to sit on is missing boards and is unstable.

Two classrooms were observed at afternoon snack time and the children did not have their hands washed prior to eating. Additionally, one classroom had also transitioned immediately from the playground and did not wash their hands after outdoor activities.

The child safety alarm is not operating correctly according to manufacturer instructions. The button at the rear of the vehicle is not disabling the alarm as it is supposed to. When inquiring about daily procedures when transporting children, the operation has not been disabling the child safety alarm at the rear of the vehicle when using the child safety alarm.

The multiple occupancy seats in the infant classroom was observed in use by the children but they were not secured while in use.
Various cots were observed at inspection being side by side and providing no walking space betweeen them. Some children were awake but some were still asleep and their cots were also directly side by side and touching.
One play yard with mesh siding had two wide holes that were greater than 1/4 inch in diameter.
A caregiver was observed changing multiple children's diapers and sanitized at the end of the diapering session instead of in between each diaper change. Non absorbent paper was not used.
