The diaper changing table in a toddler room needs to be replaced. It is missing a part of the smooth surface covering making it difficult to sanitize.

The children's products certification was last certified on 01/03/2023. This was corrected at inspection when the form was updated and posted.

Two classrooms were observed to come inside from outdoor activities and the children did not wash their hands. This was corrected at inspection when it was addressed with the caregivers and the children washed their hands.

Two infants were observed to be swaddled while laying in their crib for naptime. The sleep exception forms on file did not indicate that swaddling was medically necessary. This was corrected when it was addressed with the caregiver and the swaddles were removed.

The director's record was lacking an annual training topic.

Caregivers of the center used language not appropriate for children.

The operation did not follow suspension and expulsion policy.

The center was cited for a caregiver inappropriately grabbing a child.

Children under 5 years old were playing in open space that was not fenced in.

A wood door on playground was broken and screws were exposed. The grass was high in some areas of playground and sharp thorns were in tall grass.

One caregiver went to open front door and left another caregiver in charge of 2 classrooms which put caregiver out of ratio.

The center did not have proper loose fill depth underneath swings.

A caregiver was concentrating on the negative behavior and not offering praise or encouragement for positive behavior.

A caregiver was using language that humiliated the children.

One out of ten children's records evaluated lacked a copy of immunization record.
