This standard was tasked for the investigation. Based on the information received during the investigation, the caregiver didn't notify the parent of the child's injury that required medical attention.

It was observed during today's walk through that a caregiver didn't wash an infant's hands after a diaper change. The caregiver was told to wash the child's hands and this was corrected at inspection.

Caregiver in the toddler room left children unsupervised when she walked into the adjoining room of the toddler classroom.

Staff were observed placing infants in highchairs without washing their hands for lunch.

Caregiver in the toddler room was observed yelling at the children in the classroom when they got into an uncovered bowl of fruit left over from breakfast.

The caregiver didn't intervene and separate children in the classroom when the child was observed harming another child.

It was observed during today's inspection that a child was curled up in a ball being hit with an object by another child.

A sampling of children records were evaluated. One child's record lacked documentation regarding whether or not the parent gives permission for emergency transportation.One child's record lacked documentation that the parents received and reviewed a copy of the operational policies/parent handbook. Five children's record lacked a health statement. Three children's immunization records were observed incomplete.

A small hole that was deep was observed on the school age playground that is a potential tripping hazard.

Reduced ratio was used in the room where children younger 18 months were present. One caregiver was supervising 9 children, age 12 months, two 14 months, 17 months and 18 months to 2 year old.

In the infant room, a caregiver was caring for 5 children, age 2 months to 14 months. The classroom had two 2 months, 8 month, 12 months and 14 months. It was one child over.

This standard was evaluated in regard to an investigation and a violation was found. A 19-month-old child was left in the playground unsupervised for 2-3 minutes. The child did not receive any injury.
