During an investigation it was determined that a caregiver yelled at a child.

The operation failed to notify Childcare Regulation within the required 48-hours of a situation that placed a child at risk.

There was one child enrolled who did not have any immunizations or exemptions on file.

An infant was observed sleeping in a crib with a bottle.

There were several caregivers employed who did not have education qualifications on file.

One caregiver without a current background check on file was observed to be in a room with children. The background check was submitted during the inspection and the caregiver was removed from any classroom with children pending the eligibility results.

It was observed through video footage that caregivers were not providing adequate supervision due to completing janitorial tasks in one classroom.

It was found that the operation did not report a child injury where medical evaluation was later received.

There was no incident report filled out for a child injury requiring medical evaluation. The report was created during the inspection for parents to sign.

The cribs in the infant room did not have enough space to walk between them, This was corrected when the cribs were moved to allow for room to walk between.

One out of ten child files reviewed did not have an emergency address on the admission form. This was corrected at the inspection when the address was obtained.

The nap mats in four of the classrooms did not have a child assignment nap map available. This was corrected at inspection when nap assignment chart was completed for the classrooms.
