A child enrolled at the operation was left unsupervised on one of the playgrounds for approximately 5 minutes. During the investigation the classroom was observed and training for caregiver was reviewed and operation has met compliance following the situation.

During the investigation it was found that the operation did not notify the parents of an incident involving a child being left on the playground until 3 days after the incident occurred.

During the walk through there was no documentation for carbon monoxide testing. This was corrected at inspection when the carbon monoxide check was documented.

During the inspection a caregiver did not have documentation of SIDS, Shaken Baby Syndrome, and Early Brain Development.

Based on information obtained during an investigation, it was determined that the childcare facility did not report, within the required timeframe, a child injury requiring medical treatment by a health-care professional.

Three out of 12 children's files reviewed, needed updated immunization records on file.

One child was asleep on a cot in front of an emergency exit door. This was corrected at inspection when the child moved from the area, clearing the walkway to the emergency exit.
