One vehicle had seat cushions that had rips and tears. Some of the vehicle's seatbelt mechanisms were missing the plastic piece that attaches to the back of the seat. One seat was repaired with duct tape around the area where the seatbelt comes out of the seat.

During the investigation inspection walkthrough, the following things were observed. - the entry way floor and classroom floors had trash particles and spill stain discoloration - two garbage cans did not have lids, exposing the garbage to the children, - classrooms, bathrooms, and hallways had peeling paint - multiple tables were not sanitized after feeding leaving crumbs and other food particles on the table - used toilet paper was found on the edge of the toilet seat and on top of the toilet paper dispensers These things do not assist in maintaining the building and therefore does not protect the health of the children in care.
Children were observed watching television and eating breakfast. The caregiver was informed of screen time requirements and the television was turned off.
The restrooms were not cleaned and sanitized.

Based on information obtained during an investigation, it was determined that the childcare facility did not notify Licensing within the required timeframe regarding a situation that placed a child at risk when a caregiver grabbed a child by their shirt.

This standard was evaluated during the investigation and found out of compliance. A child was left unattended on the playground. The standard was marked into compliance at the time of the inspection when all of the caregivers were observed supervising in compliance with minimum standards. The director stated the caregiver who left the child unattended was counseled and retrained on supervision.

A plastic slide that was on the playground was broken causing a shear point in the slide.

It was discussed with the director that tears in the vinyl are not able to be sanitized properly. This was corrected during the inspection when the operation covered the vinyl with tape.
