Three infants do not have updated feeding instructions.

Two staff hired after 9/1/2023 do not have the preemployment affidavit.

Three caregivers do not have annual training in communicable diseases, and one caregiver does not have training in hazardous materials.

The operation does not have a current fire inspection on file.

The operation does not have a current sanitation inspection on file.

A caregiver was using screen time activities with children under the age of 2 years. This was corrected during the inspection when the caregiver turned the screen off.

Out of 6 employee records reviewed, 2 were missing the following: - documentation showing the date of the first day on the job - a notarized Licensing Affidavit for Applicants for Employment form - a statement signed and dated by the employee showing they have received a copyof the child-care center's operational and personnel policies - a statement signed and dated by the employee verifying the date the employee attended training during orientation

There was not a current sanitation inspection. The last sanitation inspection was completed on 12/8/21.

Out of 6 children records reviewed, 2 were missing a health statement signed by a doctor and 2 were missing hearing and vision test results required after children turn 4 years old.

One caregiver was present without an active background check status. This was corrected at inspection when the director had the caregiver leave.

Out of six personnel records reviewed, the following was observed: Five records were missing Building and Physical Premises Safety training; Three records were missing Using/Handling/Disposing of Hazardous Materials training; Two records were missing training on Responding to Food Allergies; and, One record was missing current SIDS/Shaken Baby Syndrome Training

The Director was missing the required Building and Physical and Premises Safety training.

It was found during an investigation that good judgment was not used when ankle weights were used on an infant's ankles without parental or physician consent.

Out of eight personnel records maintained, the following was observed: -Three records were missing a Notarized Affidavit of Employment; and, -One record was missing Proof of Education, a Photo ID, and Proof of Orientation completion.

Out of eight children's records reviewed, two records were missing complete emergency contact information. This was corrected at inspection, as the information was obtained and placed in the children's files.

One caregiver was present at the operation without having a background check letter of eligibility.
