Based on the gatherings during the investigation, it was determined the operation failed to notify the licensing immediately but no later than two days after a child was hurt and required medical treatment by a health care professional.

Based on the information gathered during the investigation, it has been determined that children were left unattended, and caregiver was not at a physical proximity which did not allow visual and/or auditory awareness of the children in care. Caregiver left the classroom where children were in care and children were walking in and out of the classroom.
