In reviewing medication logs for two youth, it was observed that several medication errors were not documented accordingly. Medication errors were observed on various logs in relation to missed/skipped medications, medications given at the incorrect time, and relating to wrong dosage given.

During the review of the Initial Service plan, it was found parent and child signature was missing. There was also no documentation to show the parent, caregiver, and child received the plan.
