This was corrected at inspection, when the director came up with a plan to ensure name to face checks were conducted during all transitions. One child was observed going to the restroom by itself. There were no caregivers near the restroom or standing in the hall to supervise this child. The director was informed and a caregiver was sent to supervise the child in the restroom.
