Follow up: There was a 20 month age difference in the infant room. The youngest child is 3 months old and the oldest is 23 months. Note: The ratio was adjusted to ensure the room was in ratio and there was not more than an 18 month age gap in the infant room.
The infant room was out of ratio. Two caregivers were supervising 14 children 3 months to 23 months. Leaving the room over ratio by 5 children. Note: 3 children were moved to another room and one caregiver moved to another classroom with 6 children to ensure ratio is correct.

Based on the preponderance of information obtained during the investigation there is sufficient information to conclude that caregivers either use prohibited punishment or threaten prohibited punishment by hitting the table near the child to gain their attention.
A 6 month old infant was observed sleeping in a crib with a string of beads attached to their pacifier. A photos was taken and the beads removed from the pacifier.
An infant was observed sleeping in a bouncer inside the crib. The infant was 5 months old. Note: The infant was removed from the bouncer and laid down in the crib that was bare.
Tile in the infant room is broken and cracked. Doors and walls were observed with visible dirt on them through out the building. Chairs in the cafeteria were observed with visible dirt and grime on them. Note: Pictures were taken for the file.
The electrical outlet in the infant room is missing the plastic cover exposing the inside wires and dry wall. Note: A picture was taken for the file. The cover was replaced during my inspection.
A nail was sticking out of the shelve in the toddler room. In the 3 year old room the shelve was missing the plastic boarder on the edges exposing the pressed board. Note: Pictures were taken. The nail was removed and the other shelf had tape put on the edges to keep them from getting sharp and to remove access to the wood.
The TV was on in the infant room when I arrived. Note: The TV was shut off before I left the room.

The hand washing sink was being used for both hand washing and washing bottles. Note: It was explained to the teachers that bottles could not be washed or rinsed in the hand washing sinks. A picture was taken for the file.

Three classrooms were still over ratio. Infant room: The infant room was over ratio by 7 children. 2 caregivers to 18 children when I entered the room. 7 children were removed from the room and out with another caregiver in another room. 3 year old room: The three year old room was over ratio by 2 children. An assistant caregiver arrived for the day to help in that room. 4 year old room: The room was over ratio by 3 children. An assistant caregiver arrived for their shift and was placed in the room. Note: All rooms were brought into compliance before the end of the inspection.

Daily schedules are not posted in any of the classrooms.
Three classrooms were over ratio. The infant room has 18 infants and toddlers with 2 caregivers supervising them. The infant room is over ratio by 8 children. The two year old room is over ratio by two children. One staff member is supervising 13 two year olds. The three year old classroom is over ratio by 5 children. The staff member is supervising 20 children all three years old.
Based on information obtained during the investigation there is sufficient information to prove a 15 year old was present at the operation without a background check. The 15 year old is counted in ratio for the children.
The youngest child in the infant room is four months old and the oldest child is 25 months. That is a 21 month gap.
Based on information obtained during the investigation there is sufficient information to prove that caregivers are standing in the hall way while a child goes to the restroom down the hall. Leaving the classroom unattended and unsupervised. Also, the staff are not directly supervising children in the restroom while standing in the hall by the classroom door.
There is no documentation showing the two 17 year olds are enrolled or completed the career development program at their high school.
Changes have been made to the infant room, cafeteria/school age room, and a toddler room. Licensing was not notified of these changes. Note: Pictures were taken for the file. The director informed me of the changes inside the building.

Based on the information obtained during the DFPS investigation, a child was seen on video running out of the classroom without teacher knowledge two times.
