Two infant children were observed sleeping in infant sleep sacks covering their arms and hands. This was corrected at inspection when the children were removed from sleep sacks.

There are total of 48 children in care. The operation has two tracking systems. One system only accounted for 31 children. The other system only accounted for 42 children. There were 6 children not accounted for. This was corrected at inspection when all children were signed in.

During lunch, milk was not served until directed to do so. Serving sizes were not correct for the fruit or veggies. One 20 oz can of pineapple chunks and one 14.5 oz can of whole corn were served to feed a total of 20 children. No eating utensils were provided, so children were eating with their hands. There was not enough food provided for seconds.

It was determined during this investigation that there was a large hole in the laundry room ceiling caused by previous water damage, there was damage to the paneling in the toddler room due to water damage, there was dark discoloration under the caulking in the infant room. The operation has had contractors repair the damage.
A large tree that has rotted and is dead, is adjacent to the play yard and had large chunks of bark falling of and possibly presents a risk of the tree trunk falling and causing injury to children inside and outside of the building. There was paneling observed coming away from the wall and showing signs of splintering and this appears to have been caused by water leaks. A window right below this paneling has dark substance that appears to be mold.
A substitute caregiver, left alone with children, was found to not be current with First Aid and CPR training.The substitute caregiver was also found to not be in compliance with required annual training.
The handwritten sign-in sheets did not match the electronic sign-in sheets. Neither form of sign-in sheets reflected the actual number of children in care.
Both restrooms were observed to have no means to dry hands after hand washing. The only roll of paper towels in the operation was observed to have only 4 towels left on the roll.
A substitute caregiver assigned to a group of children was unaware of the children's ages and how many children were under her care. The substitute caregiver was observed leaving children unsupervised during nap time in order to clean the cafe from lunch. The cafe and the classroom were separated by a half door.
The only milk observed in the operation was whole milk. The majority of the children in care were over the age of 2. The only current menu available for viewing was on a caregivers phone and was not able to be printed due to lack of ink in operation's printer.
A substitute caregiver was heard yelling at her own child during snack time. It could be heard by the CCR inspectors in an adjacent room.
A chain link fence in the outdoor play area was observed to have an open gap exposing sharp fencing material. A plastic play structure in the outdoor play area was observed to have broken and cracked plastic pieces causing a potential hazard to children in care. (This was corrected at inspection when the play structure was moved to an unused area).
