During a DFPS investigation it was found children in care were placed in time out for 10-20 minutes, which is 6-16 minutes longer than appropriate for the age group.

Based on evidence gathered during the investigation, a caregiver would provide time-out as a method of discipline for a four-year-old for a time of around 10 minutes.

During the course of DFPS investigation, a caregiver used a cell phone while being in charge of supervising children.

During a walk through there was an infant sleep in a playyard where the sheets were not fitted and loose.

During the walk through a child was seen leaving out of a classroom without caregiver supervision.

During the investigation it was found an incident report was not shared with a parent.

There was a walker observed in a an infant room. The walker was removed by the administration team during the inspection, bringing the deficiency into compliance.

One child in care had a documented peanut allergy on file and there was not a signed food allergy plan signed by a physician on file.

There was medication observed in an unlocked drawer without a safety lock, that could have been accessible to children. The medication was removed from the classroom during the inspection.

There were cleaning supply bottles observed in several classrooms, accessible to children. The caregiver relocated the bottles to keep out of reach of children.

There were 2 classrooms that did not have outlet covers on all outlets. The caregivers placed outlet covers on the outlets during the inspection to bring the deficiency into compliance.

A classroom was observed to be 83 degrees in the room. The room was observed to have limited airflow and did not have proper air circulation.

It was found that a caregiver offered children candy to get them to be quiet.

During a DFPS investigation, it was found a caregiver, who provides transportation to children in care, went inside of a gas station, locked the doors to the transportation vehicle, left it on with the keys in the ignition leaving 13 children alone inside of the vehicle.

During a DFPS investigation sufficient evidence was found to support children were neglected.

During the investigation it was found that a situation occurred that required immediate parental notification and the parent was not made aware of the incident until they picked the child up from care.

A child was left outside, unsupervised, after the rest of the child's class transitioned inside.

A review of the Employees Who Left Employment in 2021 data from February 23, 2022, revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022, as required.

Of the 13 children files reviewed 5 did not have current immunization records on file.

One caregiver was present with pending background checks. This subject was the only staff present in the classroom with a group of children.
