There was an exposed metal on the playground equipment that could lead to injury. There was a plant accessible to children on the playground coming from the ground that has thrones on it. On the play ground set there was a mud dauber nest on the roof. In the men's bathroom there is a hole in the big stall that can injury a child. In the 2-year-old classroom the plastic trim is exposed and can cause a tripping hazard.

The operation has not had a health inspection within12 months of their last health inspection done on August 13, 2020.
