A caregiver failed to maintain adequate supervision of a group of children when speaking to a caregiver that was in the hallway, which resulted in a child in care having their pants and pull-up to be pulled down and buttock area to be touched.

Prohibited discipline was used when a caregiver hit a child on the back of their hand. The caregiver was immediately terminated after the incident.
A caregiver did not use self-control or good judgment when grabbing a child by the upper arm to escort the child to the office who was not behaving. Two staff members did not use self-control or good judgment when the two got into an argument in the office over the discipline and guidance of a child in care who was misbehaving. The two staff members yelled at each other while the child was present in the office.
A caregiver did not use positive guidance when redirecting a child's behavior when the caregiver made the statement the child should be spanked for their misbehavior in the presence of the child.
It was found that a caregiver used inappropriate forms of discipline during naptime when patting a child's back too aggressively which resulted in red marks, holding a child's head down on the mattress to force them to sleep, and rocking cribs too aggressively while attempting to put children to sleep.
It was found that the director failed to respond to notifications about incidents that could potentially place children at risk of harm and caregivers were allowed to remain in the classrooms without further actions.
It was found that a caregiver failed to practice good judgment when using inappropriate language in the presence of children. It was also found that a caregiver failed to practice self-control when restraining a child.
There were a total of 4 classrooms with insufficient napping space between each child. There was not enough walking space between all children. There were children arranged where the caregivers were not able to properly get to children without walking over another child or children. There was one child positioned to closely to the entry doorway.
The operation was not using the individual storage designated for one classroom of children. The children were storing their belongings on the floor in the hallway when they had storage lockers to use. **This was corrected at inspection when the children's items were properly stored in lockers within the classroom.
During naptime, 1 child was arranged in front of a doorway blocking the entry/exit. 746.2909(3) During naptime, there were 3 classrooms where the arrangement did not meet the requirements of having sufficient walk and work space between each mat.
The operation failed to notify the parent immediately after a child was left unsupervised. The parent received notification 2 days after the incident occurred.

During a transition from the playground into the building a child was able to wander off into another classroom and was unsupervised for an unknown timeframe. The 2 caregivers in charge of the group of children did not recognize that the child was missing.
