This standard was evaluated in regards to a report and found deficient. A caregiver was witnessed on video forcing a child to sleep. Caregiver was witnessed putting her hands over a child's eyes and forcing him to sleep. A caregiver was witnessed patting a child on his back aggressively for an extended period of time. The caregiver was also witnessed placing rolled up mats on the child's back.

This standard was evaluated in regards to a report and found deficient. A caregiver failed to provide an alternative activity for a child when he would not sleep and continued to force him to sleep.

A caregiver was witnessed on video grabbing a child's arm and turning him over on a nap mat.

The unitary surfacing in one of the outdoor area need to be repair or replace. Some parts had broken patches and gravel was coming off the material.

The rubber edging around a play area was broken and exposed, this can be a tripping hazards.

A child was found left in a room without supervision.

A caregiver did not demonstrate competency and good judgment when not following the proper procedures during a transition, as a result a child was left in room unsupervised.

It was observed sharp and rough edges on some of the outdoor play structures.

This standard was evaluated in regards to an investigation and was found to be deficient. A caregiver failed to follow transition procedures and a 4yo child was left for 3 minutes and 15 seconds alone in their classroom.

A caregiver did not used a positive method of discipline and guidance when redirecting a child.

A caregiver did not demonstrated good judgment and competency when redirecting a child by not using positive guidance.

A child was left unsupervised in the playground.

The Facility did not report positive COVID cases within two days.
