When the active play space and equipment is inspected daily, hazards must be addressed before the children play outside each day. Evidence substantiates that children have been playing outside with hazards present. Broken and cracked toys were observed during the inspection. These were removed from the playground during the inspection. There are three poles sticking up out of the ground on the playground for the younger children where something used to be anchored. There is a loose fence board on the playground with the play structure. There is a fenced off area against the building off the deck that has a gap in it that a hand or foot could fit through.

It was observed the loose fill under the slide/climbing equipment on the playground is not at the proper depth and was very low. It was low enough that raking would not maintain the depth. More mulch needs to be added.
On the deck leading out to the playground I observed a loose board with a loose nail that needs fixed, lots of branch debris, and a an appliance with a loose cord.
A caregiver with provisional eligibility was observed alone unsupervised with children at the operation which did not meet the condition set.
During an inspection it was observed during naptime in the infant room that there were 15 children ranging from 4 months old to 18 months old with two caregivers which exceeds the child/caregiver ratio.
During an inspection, an infant was observed sleeping in a swing with a blanket.
A caregiver did not demonstrate good judgment when she picked up a child by one arm to remove him from a piece of equipment on the playground.
Caregivers were observed using their cell phones/smart watches on different occasions while caring for children.
