Based on the information provided the incident report was not shared with the parent when their child chipped their tooth while in care.

The playgrounds had overgrown weeds and grass that the children were playing in.

It was heard by Child Care Regulation a teacher yelled at a child to sit down.

There is a tripping hazard on the playground. There is an exposed pipe. A picture was taken.

Two 15 month old heads were covered with a blanket. Per the teacher this helps the children sleep. The teacher removed the blankets from their heads when brought to the attention by CCR.

On the Toddler Two Playground there is an exposed white pipe where the children run and walk around the playground. The exposed pipe poses a tripping hazard. A picture was taken.

The provider did not contact Childcare Regulation when a child was grabbed leaving red marks on their arm.

Based on the information provided a child was grabbed leaving red marks on the child's arm.

Operational policies do not address promotion of physical activities.

In the infant classroom there were 13 infants. The specified age group was 10 months. The ratio was over by 3 infants.

it was found that the diaper changing mat has a tear exposing the foam. Note: A picture was taken. This was corrected at inspection when it was replaced with a new one.

It was found a child was left unsupervised in the classroom close to an hour. Child was asleep.

It was found that caregivers did not do name to face/count when the children were moved from the classroom to gym for the movie.

Feeding instructions for 3 children not yet on table food have not been updated since August of 2022.

While reviewing 6 childrens records the following was observed: 1. One child needs an address for their school. 2. Two children need hearing and vision tests.

Cleaning supplies was hanging up in both school age bathrooms, in reach of children. NOTE: This was corrected at inspection when the cleaning supplies was moved to a higher shelf.

The first aid kit in one out of three buses was missing a thermometer, scissors and tweezers. NOTE: This was corrected at inspection when the items were added to the first aid kit.

The fire extenguisher on one bus needs to be serviced.

The health inspection was not up to date.

In the turtle classroom, the tank lid was missing.

A caregiver placed a three-month-old child in a stationary jumper; the child cannot yet support himself in the upright position therefore this is not an age appropriate activity or age appropriate equipment.

Based on the DFPS investigation video footage obtained showed a caregiver was using an electronic device while caring for children. The caregiver was looking was looking down at the device and not engaging with the children.

During the DFPS investigation it was determined that instead of holding the 4 month old child, staff propped their bottle.

During the DFPS investigation, it was determined that a caregiver failed to intervene when necessary to ensure a child's safety.

Based on the findings of a DFPS investigation it was determined that the following forms of inappropriate discipline were used, hitting, slapping, biting, pushing, grabbing and dragging a child in care.

During the investigation, it was determined that the director did not report the suspected abuse to licensing.

Based on the findings from a DFPS investigation the operation employed a caregiver who was found to be abusive in her interactions with the children in care.

During the investigation, it was determined that parents were not notified of their children being a part of the allegation.

During the investigation, it was determined that caregivers were separating children from the group for 30 minutes as a form of discipline.

During the investigation, it was determined the director did not properly supervise a caregiver allowing the caregiver to remain in the classroom after concerns were identified with the caregiver's interactions with children in care.

Two infants had bottles in their cribs. They were sleeping. This was corrected when the bottles were removed.

An employee stated she had 14 children then stated 13 children. It was verified by Licensing there were 14 children in care.
