Two out of two home records reviewed did not mention the required information.

One out of two files were not completed per minimum standards requirements. Placement agreement from DFPS website was used however it was not completely filled out.

Child in care received medical treatment for a burn. This was reported after the 24-hour required reporting timeframe.

The children in care reported they are told to write lines and are given a set time as punishment. If not completed in the designated time, more pages are added. Both children reported hand/wrist pain from writing lines and their work is thrown out following completion.

Foster parent admitted to tapping children in care with foam pool noodle to redirect children in care. Foster parent admitted to pulling child's shirt collar to redirect child in care.

The operation was unable to provide complete medical records for Licensing's review. All medical records have not been uploaded to electronic system.

Child in care sustained a humerus fracture, when foster parent placed child in car seat.

During the investigation it was found during the interviews with children in care the foster parent threw the child on the bed as a form of punishment, which caused pain to the child's back.

During walk-through of home, there were various cleaning substances stored in areas where children could access them and accidentally ingest them.

During walk-through inspection, there were various medications/supplements not in locked containers and/or easily accessible to children.

A child who received emergency medical attention was not reported to licensing within 24 hours.

Adult family member who resides in the home did not have an initial background check completed.
