During an investigation, it was determined that a caregiver knowingly allowed children to remain outside of visual and auditory awareness while attending to other duties in the classroom.
Based on the interviews conducted, a caregiver did not conduct a name to face while lining the children up inside the classroom and when arriving outside before the children start to play to ensure all children were accounted for. This resulted in a child being left in the classroom unsupervised for approximately 12 minutes. Based on the interviews conducted, five children between ages 18 months and 2 years old were left on the playground by a caregiver with another caregiver watching the children from a distance from a classroom in the doorway.
Based on the interviews conducted a parent was not notified immediately when a child was left in a classroom unsupervised. The parent was notified at pick-up.
