A five month old infant was observed sleeping on the floor on a Boppy pillow with a blanket and a soft plush toy. This was corrected at inspection, as the infant was no longer sleeping on the Boppy pillow.
Based on interviews conducted with operation staff, collateral parent contacts, and an inspection at the operation it was found the operation denied parents entry inside the building to observe their children in care.
During the walk through inspection the following was observed in the toddler room: a toddler was observed standing on top of a table and climbing underneath the changing table. The toddlers were observed running around the classroom not participating in the activity listed on the daily schedule.
During the walk through insepction the infant room was observed to only have 4 out of 10 cribs for the non walking infants younger than 12 months.

During the inspection a child was observed walking from one classroom to another classroom without adult supervision. This was corrected at inspection, the director placed the child back in thier classroom and addressed the issue with the caregiver. The director will ensure Positive Guidance and Discipline, and supervision training will be provided and complete by each caregiver by 06/21/2021

During the walk through inspection three classrooms were observed out of ratio: 1. The infant classroom child/caregiver ratio was 1 caregiver to 5 children 0-11 months of age, the classroom was observed to be 1 infant over the ratio 2. The toddler classroom was observed to have 1 caregiver to 18 children, the specified age group was 2, this classroom was over by 7 children 3. The Pre-K classroom was observed to have 1 caregiver to 28 children, the specified age group was 4 years old, this classroom was over by 10 children
During the inspection children jackets were observed stacked on top of one another and not placed in each child individual storage. This was corrected at inspection, all jacket was placed in each child's individual storage area.
During the walk through inspection a bleach spray bottle was observed on a low shelf accessible to children in care. This was immediately corrected at inspection, the bleach bottle was moved to a high shelf inaccessible to children in care.
