The crib did not contain a firm flat mattress. This was corrected at the inspection when the director put a mattress and firm fitted sheet into the crib.

A caregiver with a pending background check was observed in a room with children.

An infant was observed sleeping in a bouncer.

During the inspection, there was no caregiver present in the infant room.

During the inspection there was a person present with direct access to the children before their FBI fingerprint results were in. The person was being counted into ratio.

During the monitoring inspection, 2 infants were observed in a restrictive device for more than 15 minutes.

During the inspection, a caregiver was observed texting on their cell phone during the children's outdoor play time.

During the inspection 1 of personnel records reviewed did not contain the 24 hours of annual training required.

During the inspection a sharp metal board was observed sticking out from the side of the building on the playground area. This was corrected at the inspection when the caregiver placed duct tape around the sharp areas.

A review of the Employees Who Left Employment in 2022 data from January 26, 2023, revealed that the center had not reported the number of employees who left employment in 2022, as required.
