During the follow up, a caregiver was observed serving green beans on children's plates with a gloved hand instead of a serving utensil.

During the follow up inspection, it was determined staff are unaware of the procedures if second servings of food are needed by children.

Based on information gathered, the operation does not have adequate food on hand to provide meals to every child enrolled at two days out of the week. Based on information gathered the operation does not provide alternative meals for children when they do not bring food from home or do not want to eat the food being served. Based information gathered operation does not follow the meal patterns established by the USDA and CACFP.

Based on information gathered, it was determined the operation encourages parents to provide meals and snacks for children enrolled, but has not included a statement in its enrollment agreement and has not added an addendum explaining that if a parent is choosing to provide meals and/or snacks from home, the operation is not responsible for its nutritional value or meeting daily food needs of the child.

Based on the information gathered, it was determined that the operation used food as a reward to achieve desired outcome.

A caregiver supervising infants has not received required training to care for children under the age of 24 months.

The operation did not notify Child Care Regulation of an injury of a child in the care of the operation that required treatment by a medical professional within the required two day time fame. The operation was notified of the injury on 3/28/2022 and did not notify licensing until 3/31/2022.

Based on information obtained, it was determined that caregivers conduct administrative and janitorial duties while supervising children.

Children were observed eating food off of a bare table.

The operation did not notify Licensing or all parents about positive Covid cases of staff and children.

A caregiver with no pre-service training was observed supervising children.

Based on information obtained, the center has been out of ratio three days out of one week.

The director placed an employee with zero annual training in classrooms to supervise children.

One child at the operation with a known allergy does not have a food allergy plan.

The operation has two epinephrine injections and a bottle of benadryl that is on file and expired.

The operation's director has been out on leave since the beginning of February 2021. The operation did not report this information to licensing.

The operation's Infant I and Infant II classrooms were observed to be out of ratio. This was quickly corrected at the time of the inspection.
