On the outside facing the school age playground, there is a window security bar hanging off the window and pieces of the gutter are on the ground. In the indoor play space there are broken plastic windows, a broken border piece, and walls with chipped paint.
Three childrens files do not have updated immunization records.
On all three playgrounds, the play stucures platform coatings have broken off or torn off. On the school age playground equipment, the walking bridge has broken pieces. On the toddler playground there was a broken toy car.
There is a tear in the back of a seat on one of the centers vehicles.
The children's nap mats in the classrooms were not labeled to each individual child at the center.
