Inspector observed one caregiver on their cell phone while caring for children. Inspector spoke to staff who stated they are not allowed to be on their cell phones when in a classroom with children. Inspector has previously provided technical assistance on staff on cell phones while caring for children during a previous inspection.
Inspector observed one person present and in a classroom with unsupervised access to children without an initial background check submitted. At the time of inspection the caregiver was seen in multiple classrooms.
Inspector observed two classrooms out of ratio. The infant room had 11 infants with one caregiver. The caregiver did not know the ages of the infants except that the youngest infant was one month old. A second classroom had one caregiver with 23 children. The children were between 2-4 yrs. old. The specified age group was 3 yrs. old.
Based on the information provided during the course of the investigation, two infants were observed being left asleep in car seats while in care at the operation.
The operation did not have annual documentation for checking the carbon monoxide, smoke detector, or fire extinguishers monthly.
The emergency preparedness plan was missing information such as how children with limited mobility or who are under 24 months will be evacuated, and how staff will continue to care for children until parents arrive.
Inspector observed two classrooms change children and not sanitize the diaper changing table after each child.
Inspector observed one 4 month old infant in a crib lying on a boppy pillow.
One staff left classroom to open the door leaving the classroom out of ratio. There was one staff with 8 infants until the second staff returned.
Inspector observed one 6 month old infant in a bouncer with a blanket and bottle. Staff stated the blanket was being used to prop bottle.
Several classrooms were observed to have chipped paint on walls which is accessible to children.
A caregiver in the infant room was observed feeding a child with bottle while child was sitting in a bouncer seat.
Two stationary play structures (one blue Thomas the Train and one red/pink fire truck) was observed with broken plastic parts. The broken plastic was observed to be sharp with jagged edges which poses a risk to children.
Two out of six classrooms observed did not have an activity plan posted.
One child, age five months old, was observed sitting in a bouncy seat for almost thirty minutes.
One child was observed awake sitting in a stroller during naptime. NOTE: this was corrected at time of inspection.
A caregiver was observed talking to another caregiver through the door to which her back was turned toward the children resulting in one child crawling out the doorway and into the hallway.
In one classroom, childrens' snack was not placed on a plate or napkin prior to serving.
