Kids Place at Grace

Based on the information gathered by the DFPS investigation, a caregiver grabbed and threw a child on a table, causing bruising to the left and right arms, left shoulder and left lateral torso.

The standard was found deficient as part of a DFPS investigation.

Based on the information obtained during the course of the investigation, one child was not properly supervised and as a result left the operation and went into a high-traffic road. At the time of the incident, four staff were observed in close proximity to one another instead of caring for children.

One staff did not communicate with other staff about which incoming children they would be in charge of.

Staff used lollipops to bribe a child who was attempting to climb the fence and later left the operation unsupervised.

At the time of inspection on 4/9/24 the operation was over their infant capacity. The operation's capacity for infants is 23 and they had 24 infants in care.

At the time of inspection the operation was using one room as an infant room without the approval of an amendment. The room has been used since March 2024 for infants however an amendment was not submitted until April 2nd, 2024 and has not yet been approved.

Inspector observed two staff in two different classroom on cell phones. This citation was previously cited for an investigation. This is a re-cite.

The emergency preparedness did not have information on how children who are under 24 months or who have limited mobility will be evacuated or for how staff will continue to care for children until parents arrive.

The operation failed to submit and receive the results of a background check and FBI Fingerprint for one person who was present and had unsupervised access to children.

Four out of four staff did not have complete training records on file. One out of four staff was missing training on Sids, Shaken Baby, and Early Brain Development, Food Allergies, and Emergency Preparedness. One out of four staff was missing six hours of training including child maltreatment. The caregiver needs in person training on file. One out of four staff had no trainings on file and requires pre-service. One out of four staff was missing training in child maltreatment, sids, shaken baby and early brain development, and food allergy.

The operational policy on file was missing the following information: 1. Information on TB testing for children and adults 2. Transportation 3. Maintaining unassigned eppi-pens 4. Promotion of indoor/outdoor activity 5. Procedures for parents to contact the director for any concerns, or questions

Inspector observed two infants asleep in restrictive devices. One infant was four months in age and one was six months in age. One child was in a swing and the other was in a bouncer. Inspector observed the classroom a second time later during the inspection and an infant was again observed asleep in a swing.

Inspector observed one infant 5 months of age asleep on stomach, when inspector spoke to staff they stated they placed the baby on her stomach to sleep.

There are no fire drills listed as of January 2024. The operation also has no documentation of checking the carbon monoxide, fire extinguisher, or smoke alarm.

Based on the information provided during the course of the investigation, staff were texting on cell phones while caring for children.

Based on the information obtained during the course of the investigation, staff at the operation failed to report suspected abuse/neglect within the operation.

Operation did not have a current fire inspection at time of follow up inspection. This deficiency will be recited.

The outside playground area did not have the required six inches of loose fill around the play structure which also has a slide.

Two infant classrooms observed during lunchtime were not utilizing the safety straps on high chairs, half-moon bucket seats, and low-to-the-floor high chairs.

One out of the two transportation vehicles did not have an electronic child safety alarm installed in the vehicle.

Operation did not have a current fire inspection; the last fire inspection was completed on 4/8/2022.

At the time of inspection, a caregiver who had unsupervised access to children in care, was present with an ineligible background check.

One child's food allergy emergency plan was not signed by the parent.

There is peeling paint and rust on a stationary piece of equipment used by children for climbing and sitting. The toddler play area has a gate that does not oepn freely as the bottom of the gate scrapes the ground.

The outside playground area did not have the required six inches of loose fill around the play structure which also has a slide.

At time of inspection, the director did not have required transportation training on file.

At time of inspection, personal belongings, such as backpack or purse, was not properly secured and was accessible to children. NOTE: This was corrected at time of inspection.
