Inspector observed a second caregiver reading a book while caring for children who were napping and a third caregiver eating lunch while in a classroom with children who were napping. Inspector observed one 9 month old infant asleep with a swaddle outfit on top of him being used as a blanket. Inspector asked that staff remove it from the crib and staff member put it in the corner of the crib.

Based on the information gathered by the DFPS investigation, a caregiver grabbed and threw a child on a table, causing bruising to the left and right arms, left shoulder and left lateral torso.
The standard was found deficient as part of a DFPS investigation.
Based on the information obtained during the course of the investigation, one child was not properly supervised and as a result left the operation and went into a high-traffic road. At the time of the incident, four staff were observed in close proximity to one another instead of caring for children.
One staff did not communicate with other staff about which incoming children they would be in charge of.
Staff used lollipops to bribe a child who was attempting to climb the fence and later left the operation unsupervised.
At the time of inspection on 4/9/24 the operation was over their infant capacity. The operation's capacity for infants is 23 and they had 24 infants in care.
At the time of inspection the operation was using one room as an infant room without the approval of an amendment. The room has been used since March 2024 for infants however an amendment was not submitted until April 2nd, 2024 and has not yet been approved.

The operation failed to submit and receive the results of a background check and FBI Fingerprint for one person who was present and had unsupervised access to children.

Inspector observed two infants asleep in restrictive devices. One infant was four months in age and one was six months in age. One child was in a swing and the other was in a bouncer. Inspector observed the classroom a second time later during the inspection and an infant was again observed asleep in a swing.
Inspector observed one infant 5 months of age asleep on stomach, when inspector spoke to staff they stated they placed the baby on her stomach to sleep.
There are no fire drills listed as of January 2024. The operation also has no documentation of checking the carbon monoxide, fire extinguisher, or smoke alarm.
