Several play yards used for infants were observed to be dirty along the top rails of the device.

An infant was observed sitting in a high chair that did not have the safety strap securely fastened. NOTE: This was corrected at time of inspection when the caregiver fastened the child safety strap.

In two classrooms, children were placed on a nap mat that was not assigned to them based on the numbered chart.

Several classrooms have chipped and/or peeling paint on walls which is accessible to children. Several classrooms had holes in the walls. In one classroom, masking tape was placed over a hole and is surrounded by peeling paint.

Based on the DFPS investigation, a child was left outside unsupervised for approximately one hour.

Based on the DFPS investigation, operation did not designate a person in charge when designated director was absent from the center.

Based on the DFPS investigation, parent was not immediately notified of a situation that placed a child at risk. Operation was aware at 1pm that a child was left outside unattended; operation did not notify parent until approximately 3pm.

A six month old infant was viewed drinking a bottle in a bouncer.

Three out of seven infant classrooms had six infants with one caregiver. Later at inspection the director put the three infant rooms into compliance.

The operation administered an ointment medication to a child one year after the expiration date.

Inspector observed the play area with exposed cement. The cement was holding poles in place that is used for shading.

The director did not have the required 30 hours of annual training on file for 2019-2020. The director was missing 18 hours of annual training. Training areas missing included Abuse and Neglect, SIDS, Shaken Baby, and Early Brain Development, Management, and trainings in section (e) of this standard.

Inspector observed one infant approximately 5 months in age asleep in a bouncer. Staff stated the infant had been asleep for approximately 20 minutes.

Inspector observed one staff wiping two children's noses and did not wash hands in between children.

One out of four staff did not have most of the required trainings on file for this standard. The staff member had communicable disease training on file.

Bottles and cups at the facility did not have the child's first name and last initial. Staff use a ribbon on the bottles however one child was observed putting the ribbon in his mouth.
