Several play yards used for infants were observed to be dirty along the top rails of the device.
An infant was observed sitting in a high chair that did not have the safety strap securely fastened. NOTE: This was corrected at time of inspection when the caregiver fastened the child safety strap.
In two classrooms, children were placed on a nap mat that was not assigned to them based on the numbered chart.
Several classrooms have chipped and/or peeling paint on walls which is accessible to children. Several classrooms had holes in the walls. In one classroom, masking tape was placed over a hole and is surrounded by peeling paint.

Inspector observed one infant approximately 5 months in age asleep in a bouncer. Staff stated the infant had been asleep for approximately 20 minutes.
Inspector observed one staff wiping two children's noses and did not wash hands in between children.
One out of four staff did not have most of the required trainings on file for this standard. The staff member had communicable disease training on file.
Bottles and cups at the facility did not have the child's first name and last initial. Staff use a ribbon on the bottles however one child was observed putting the ribbon in his mouth.
