In the toddler room, there was insufficient number of toys and equipment accessible to children. Toys, books, and puzzles are stored on high shelves out of reach of children.
Two out of the five cribs in the infant room did not have tight, snug fitting sheets.
The outside play area had two pieces of stationary play equipment--one is an apparatus that has bicycle wheels and the other is a stationary bouncy on springs---both pieces of equipment have peeling paint and rust. Also observed overgrown grass underneath the play equipment and along the fence line of outside play area. There were several ant piles observed on the outside play area.
A five year old child was sleeping in the hallway during naptime. The child was separated from the group and the caregiver.
A caregiver was observed changing two infant diapers and did not wash the children's hands after diaper change.
One classroom had a pair of adult scissors, bug spray, tape dispenser, and a bottle of children's Ibuprofen stored in an unsecured cabinet accessible to children. This was corrected at inspection.
A child's whereabouts were unknown for approximately two minutes when child was found unsupervised in the restroom.
Operation did not notify parents in writing that an animal was present during operating hours.
