In one of the classrooms, garbage was made accessible to the children by not having a lid on the Trash Can. This was corrected when the director placed a lid on the trash can.

Based on the information gathered, a caregiver did not share the child's food allergy information with the new assigned caregiver.

Based on the information gathered and preponderance of evidence, a child's EpiPen was expired as of March 2021, and documented on the Medication Authorization Form dated 2/18/20 - 3/18/21.

Based on the information gathered, the child's food allergy was not posted in his newly assigned classroom.

One caregiver had two classrooms for a total of 23 children ages 4 and 5 years. The second caregiver arrived within a few minutes to separate the 2 classrooms. The correction was made during the inspection.
