There was not a shower door/curtain on the lower level shower. The children were having to using the shower on the main floor, which is not where their bedrooms are located.

A staff person was "horse play and rough housing" with the children in care and an injury occurred.

Two incident reports did not have the age and gender of all children involved. In addition, the following was also missing. All witnesses were not listed, as well as the resolution of the incident was not document.

A caregiver left 4 children in a van without any supervision on 2 occasions.

Two of the children's service plan required "constant" supervision; this was not followed.

Two children 13 and 10 years old were left unsupervised by a caregiver in a vehicle and engaged in inappropriate sexual behavior.

There were two employees that had expired training in Normalcy. The last date the training was taken was 3/6/22.

Medications skipped or not filled in a timely manner were not documented as required for all prescribed medications.

Incident reports provide insufficient information on interventions before and after an incident and it does not state who spoke to the child/children in care.

Staff did not administer medications to children in care as prescribed by the physician.

There was mold along a wall in a child's sleeping area. In addition, there was a broken toilet lid in the children's bathroom.

The count on a medication log that was reviewed did not match the count of medications in the bottle. There were 30 pills in the bottle and there were 27 pills documented on the medication log.
