The operation had not conducted a fire drill since October 30, 2025. The operation conducted a fire drill during the inspection and documented it, bringing the operation into compliance.

The operation has a daily and monthly checklist however they are not utlizing this and keeping documentation of this. The last checlist for buildiing, grounds and playground was completed in October 2023. This was corrected at inpection when Director completed a thorough walk through of the operation and documented this using the building and grounds checklist.

The daily sign in sheets shows 5 children in the toddler group, and 7 on the preschool class. There were only a total of 11 children in care. The sign in sheets for the 13th were missing for the preschool class.

During this investigation, it was determined that the operation is routinely exceeding the room capacity of 7 children. This was corrected during inspection dated 4-6-23, when the operation was advised of the rules regarding room capacity.
The operation did not have a policy in place that all caregivers knew and were following for taking children outside based on weather conditions. Therefore, children were not being provided proper outdoor play opportunities as shown on the activity plans.
A tv was on a shelf in a classroom and was not achcored. This was fixed during the inspection when the tv was removed from the classroom.
Based on observations and interviews with staff, activity plans are not being followed.
It was determined that 2 school aged children, who are homeschooled are at the operation daily, and are allowed to be in the school age classroom unsupervised on a regular basis.
Based on records reviewed, staff's pre-service training did not meet the requirements of mimimum standards. What training was provided, was presented by a person not qualified to present the training per 746.1317.
During the investigation it was determined that the operation had a caregiver providing unsupervised care while the caregiver has provisional conditions attached to the background check.
As a result of this investigation, it was determined the operation knowingly hired an employee as a caregiver who did not have a high school diploma or GED as required in 746.1107, and allowed the employee to perform caregiver duties with and without supervision of a qualified caregiver. The operation created a high school diploma in the caregivers name to keep in the file in an attempt to satisfy standard 746.1107 pending the caregiver meeting standard 746.1107 at a later date.
A caregiver did not have a high school diploma, GED or certificate of completion. This was cited under 746.201(9). The caregiver was sent home pending completion of this requirement.
