It was determined that a caregiver did not exercise good judgment when choosing to roughhouse with children which resulted in an accidental injury of a child.

Based on the investigation findings, it has been determined there needs to be more supervision due to the frequent bites observed on the victim. No caregivers could intervene in a timely manner to prevent the biting.

The Children's Product Certification Form was not available at the time of the inspection.

The emergency plans for 2 children with diagnosed food allergies, lacked the required signatures. Both children have EpPens. (1 lacked the parent signature & the other lacked both the parent and health professional signature).

It was determined that staff left a child unsupervised for a brief period - as they transitioned the group from one area to another.

The caregivers admitted to leaving a child alone in a classroom due to not conducting a name to face check before going outside or checking the classroom before leaving the classroom.

The operation did not report an incident where a child was injured and required medical attention within the 48 hour time span.

The operation did not have a current health inspection.
