During the inspection the fire inspection report provided was from 2021 and not the most recent inspection report. Note: During the inspection the most recent report was not available for review.

During the inspection, I observed a phone charger cord accessible to children in care. I also observed on the playground two metal poles accessible to children in care.

During the inspection an infant was found sleeping in a restrictive device.

During the inspection, I observed changing equipment/pad on a table used by children in care.

During the inpsection infants were not in the desginated crib.

During the inspection the operation failed at creating a safe and sanitary condition for children in care as it pertained to an animal.

During the inspection an infant was found with a blanket over their head, the infant was completely covered in the restrictive device. Note: The regulatory inspector removed the blanket from the infants head and body and informed the Designee by phone.

During the inpsection the caregiver left two infants unspervised to answer to front door.

During the inspection garbage/trash can was accessible to children in care.

During the inspection, two infants files were incomplete and missing the required emergency information.

During the inspection caregivers did not who was in charge in the absence of the primary and second director. During the inspection other caregivers gave different names of who was in charge.

During the inspection a stray cat was found in a vehicle used to transport children. Note: The doors were opened by the second director and the cat ran out.

I observed an infant upset and crying while caregivers were on personal devices. Note: The caregiver put away all personal devices and immediately attended to the infant when instructed.

I observed a rodent bait trap accessible to children in care. The rodent bait traps were removed and were inaccessibe to children.

I observed four infants in care under the age of two years old engaging in screen time during the inspection. Infants were relocated to the infant room.

I observed a seven month old diaper full who had not been changed promptly. The caregiver was instructed to change the infants diaper.

I observed the infant classroom floor space filled with items/objects the director mentioned were used as floating items/objects. Items included travel shelves, cots, and bowling balls and pins. Note: The area was cleared of all hazards before the inspection concluded.

I observed a water hose accessible to children, near the door that leads to the playground. Note: The water hose was put away during the inspection.

When asked during the inspection, caregivers did not provide licensing with an activity plan.

I observed a caregiver on a personal device while at the operation during the inspection. The caregiver removed the device during the inspection.

The operation has not done their annual fire drill for 2021.

The operation has not documented their monthly fire extinguisher inspection for 2021.

The operation has not update their children product certification since 2020.
