The caregiver did admit to using her cellphone while supervising children

All playground areas has not been maintained with loose filled.

2 out of 3 children who receives an epi pen does not have a food allergy emergency plan.

The operation had 2 expired epi pens of children in care. The director placed the epi pens away and contacted the parents to come and pick them up.

There were two classrooms that had cleaning spray on the counter which is accessible to the children. Director did removed the cleaning sprays and placed them in a cabinet.

Caregiver did not know the name of the children in her classroom. Caregiver did not have a list of the children in her classroom.

The operation did not provide a copy of the incident report on the day it occur to the parents within a 48 hour period. The parent did not sign and acknowledge the incident report either.

The operational staff did not use good judgment and interfere when a child was put on a potential risk to injury while he was in care.

The operation does not have a current annual fire inspection since 10/30/2019.

The operation is not maintaining the loose-fill in the toddler playground. The playground surface can be seen and not being maintain at six inches required by licensing.

The operation has not update their children product certification since 2020.

The operation fail to notify licensing that two operational staff test positive for COVID-19 on a timely manner.
