Based on information obtained I observed a caregiver pulling a child to the ground and holding in a restraint hold.

Caregiver did not have self-control when assisting with the child?s behavior.

A caregiver pinched a child's nose which resulted in a bruise left on the child's nose.

Based on information obtained I observed a caregiver pulling a child to the ground and holding in a restraint hold.

Based on an investigation it was determined a child in care was physically abused.

A review of the Employees Who Left Employment in 2020 Survey data from January 25, 2021, revealed that the center had not reported the number of employees who left employment in 2020 by January 15, 2021, as required.

During an investigation there was sufficient evidence that a caregiver inappropriately disciplined a child when the caregiver swung the child by the arm.

A review of the Employees Who Left Employment in 2021 data from February 23, 2022, revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022, as required.

The caregiver failed to documented the incident report when the child was injured in the pre-k playground.

A walkthrough was conducted. Three classrooms were observed and seven sinks were not working properly. Seven children sinks did not have running water. In pre-school playground, the gate was not properly secured. It is missing a gate lock. In the toddler playground, the gate was not properly secured. It is missing a gate lock. In the afterschool playground, two fence was not being properly maintained. The fence bar was seen torn on each side.
