The parents rights information in the operational policies was incomplete.

A caregiver did not know the correct number of children present in their classroom during the inspection. The caregiver stated there were 17 children when there were actually 16 children physically present in the classroom.

1 child's file did not contain the address for the emergency contact person.

Staff's files evaluated did not contain documentation showing training was completed in the following topics: Handling Storing and Disposing of Hazardous Materials Understanding Building and Physical Premises Safety Understanding and Responding to Food Allergies Emergency Preparedness

During the walk through, a 14 month old infant was observed sleeping on a nap cot with a blanket covered their head. This was corrected when the caregiver was made aware and the blanket was uncovered from the child's head.

During a walkthrough, it was observed a 14 month old was inappropriately handled during nap time. The infant was observed to be pick up by their arms and brough to their nap area inappropriately.

One employee files was observed to be not current with their annual training.

The current annual training certificates for the director were not available for review. The last training records were observed to be completed in 2023.

One caregiver from the infant classroom was observed present without having the completed background check determination.

A child was injured and required medical attention while playing on the playground.

Based on the information gathered during the investigation, the operation was cited due to a child in care being bit by ants while in care in the operation.

One of two transportation vehicles electronic safety alarm did not working during the inspection.

One of seven infants obsevered in a classroom did not have the updated feeding schedule renewed/signed by the parent.

Based on the information obtained during this investigation the operation was not in compliance as it pertains to this minimum standard citation.

A caregiver walked to the front desk area and left a child unattended in a classroom. *The caregiver is no longer employed at the center.

A child's was observed sleeping on a cot positioned in front of an exit during naptime. *This was corrected during inspection whencaregivers repositioned the childs cot so that it did not obstruct the exit door.

A child was observed reclining in a bouncer while drinking a bottle. *This was corrected during inspection when a caregiver removed the child from the bouncer to hold during feeding.

Annual transportation training has expired for one driver.
